Getting started with Heritage Human Services, Inc. begins by completing the enrollment requirements for the program you wish to receive. To enroll in the Group Supplemental Day Habilitation Program, Community Prevocational Services, Community Habilitation Program, Respite Service Programs, Behavior Management Training, Individual Skills Training, or another eligible program, submit the required documents below and complete the enrollment form to begin the process.

Life Plan
Current Life Plan with Heritage Human Services listed as the designated program Provider.
Level Of Care (LOC)
Most recent, updated Level of Care documentation.
Clinical Evaluations
Comprehensive Psychological and Psychosocial evaluation reports.
Physical Examination
Recent physical examination results including Tuberculosis (PPD) test results.
IEP Documentation
The most current Individualized Education Program (IEP) document, if applicable and available.
